Individual 30-minute review of a student’s treatment plan, goals, and objectives
Welcome and thank you for joining this one-on-one Plan Power-Up session. Briefly introduce yourself and state that the goal is to ensure a shared understanding of the student’s treatment plan, goals, and objectives for seamless implementation.
Session Objectives
- Ensure clarity on the student’s identified needs and selected interventions
- Confirm that each goal and objective meets SMART criteria and aligns with interventions
- Identify any gaps or refinements needed
- Agree on immediate next steps and follow-up
Walk through each objective aloud so the clinician knows what we’ll accomplish by the end of the session.

Open the Student Treatment Plan Document. Guide the clinician through each section: student background, present levels of performance, chosen interventions, and service frequency. Pause to clarify terminology or processes.
Goals & Objectives Alignment
- Review each goal against SMART criteria
- Confirm objectives are measurable and tied to interventions
- Identify gaps or suggest refinements
Display the Goals & Objectives Checklist. For each goal, verify SMART elements: Specific, Measurable, Achievable, Relevant, Time-bound. Ensure objectives link to interventions and discuss any refinement.
Reflection & Action Planning
- Complete reflection prompts on clarity and questions
- Note suggested action steps and responsible person
- Set implementation timeline
Hand over the Reflection Prompt Sheet. Invite the clinician to jot down areas of clarity, outstanding questions, or action ideas. Lead a brief discussion to finalize next steps and timelines.
Wrap-Up & Next Steps
- Review agreed action items and timelines
- Confirm date and format of the next follow-up session
