Students will design a concise 5–7 question mental health screener targeting key domains to triage referrals into Tier 2 group or Tier 3 one-on-one counseling.

A brief, focused screener enables quick identification of student needs and directs them to appropriate district mental health supports without delay.

Step 1

Introduction

2 minutes

  • Explain the goal: create a brief screener to triage student referrals
  • Highlight importance of clear, neutral, time-bound questions
  • Show the rubric and explain its role in self-assessment

Step 2

Identify Domains

3 minutes

  • Review common mental health areas (e.g., mood, anxiety, trauma)
  • Ask the student to select 3–4 priority domains for the screener
  • Have them note why each domain is critical for triage

Draft Questions

  • Student writes 5–7 concise questions, each aligned to a chosen domain
  • Ensure neutral language, specify a timeframe (e.g., ‘past two weeks’)
  • Refer to rubric criteria: clarity, sensitivity, domain alignment

Step 4

Self-Check with Rubric

2 minutes

  • Student uses the rubric to evaluate each question on clarity, relevance, and tone
  • Identify any questions needing revision and adjust accordingly

Step 5

Finalize Screener

2 minutes

  • Organize questions in logical order with clear instructions
  • Add a brief introduction explaining purpose and timeframe
  • Save or share the finalized screener with the counselor
