Mystery Scenario Setup

Students will engage as ‘note detectives’ to analyze counseling progress note excerpts, identify underlying student needs, and accurately document their findings using a structured note format.

This lesson strengthens critical reading and professional documentation skills by having students decode real counseling notes, fostering analytical thinking and clear communication—key competencies for effective support and collaboration in MTSS Tier 1.

Prep

Teacher Preparation

15 minutes

  • Review all generated materials: Clue Presentation Slide Deck, Evidence Stations Activity, Clue-to-Note Mapping Worksheet, and Note Analysis Criteria Rubric.
  • Print and cut station clue cards for each evidence station.
  • Arrange station materials around the classroom in logical order.
  • Load the slide deck on the classroom projector and test audio/visuals.

Step 1

Warm-Up Discussion

5 minutes

  • Pose the question: “Why are accurate progress notes essential in counseling?”
  • Brainstorm key elements of effective notes (e.g., observations, interventions, outcomes).
  • Record student ideas on the board to refer back to during the activity.

Step 2

Mystery Scenario Introduction

5 minutes

  • Present the case overview using the Clue Presentation Slide Deck.
  • Explain students’ roles as detectives decoding note excerpts to reveal a student’s needs.
  • Model analyzing one sample clue and collaboratively identify the need and note component.

Step 3

Evidence Station Rotations

25 minutes

  • Divide students into groups of 3–4.
  • Assign each group to begin at a different Evidence Stations Activity station.
  • At each station, groups review a progress note snippet and related clue cards.
  • Record observations and hypothesized needs on the Clue-to-Note Mapping Worksheet.
  • Rotate groups every 6–7 minutes until all stations are visited.

Step 4

Group Share & Note Mapping

10 minutes

  • Reconvene as a class.
  • Invite each group to share one key clue and the student need they identified.
  • On the board, collaboratively map each clue to its corresponding need, using language from the rubric.

Step 5

Debrief & Self-Assessment

5 minutes

  • Distribute the Note Analysis Criteria Rubric.
  • Students compare their worksheet entries against rubric criteria and self-assess accuracy.
  • Facilitate a discussion on how using the rubric improves consistency and thoroughness in documentation.

Welcome everyone! Introduce today’s activity and set the tone: we’re going to become Note Detectives, deciphering counseling notes to reveal student needs.

Mystery Scenario Overview

Meet Jordan, an 8th grader who has been:
• Missing class more often than usual
• Appearing withdrawn during group work
• Turning in assignments late

As Note Detectives, our mission is to review counseling note excerpts and identify what Jordan needs.

Explain the mystery scenario: introduce the student case and why we need to investigate. Emphasize relevance to real counseling work.

Key Elements of Progress Notes

  1. Observations: Factual descriptions of behavior
  2. Interventions: Strategies or supports provided
  3. Student Response: How the student reacted or participated
  4. Plan/Next Steps: Recommendations or follow-up actions

Highlight the core components of a high-quality progress note. Use this slide as a reference throughout the activity.

Sample Clue Walkthrough

"On 9/14, Jordan clenched fists and avoided eye contact when asked about homework stress."

• Observation: clenched fists, no eye contact
• Possible Need: coping skills for anxiety
• Note Component: Observation + Intervention suggestion

Model analyzing one clue. Read the snippet aloud, then walk through how to pick out the observation, infer the need, and map it to the note component.

Station Activity Instructions

• Form groups of 3–4 students
• Begin at your assigned Evidence Stations Activity station
• Review the note excerpt and clue cards
• Record your observations and hypothesized needs on the Clue-to-Note Mapping Worksheet
• Rotate every 6–7 minutes until all stations are complete

Explain how students will work in groups, rotate stations, and use the worksheet. Point out timing and expectations for each station.

Evidence Stations Activity

Overview:

In this hands-on rotation, students will visit four stations, each featuring a counseling progress note snippet and three clue cards. Working in groups, they’ll analyze observations, infer student needs, and record their findings on the Clue-to-Note Mapping Worksheet.


Station Setup & Rotation Instructions (25 minutes)

  1. Arrange stations 1–4 around the room with printed materials at each station.
  2. Divide students into 4 groups. Assign each group a starting station.
  3. At each station, assign group roles:
    • Reader: reads the note snippet aloud
    • Analyst: identifies key observations
    • Hypothesizer: infers the student need
    • Recorder: fills in the worksheet
  4. Groups spend 6 minutes per station.
    When the timer rings, rotate clockwise to the next station until all four are complete.

Stations Details

Station 1: Social Withdrawal

Progress Note Snippet:
"On 10/5, during lunch period, Jordan sat alone, avoiding eye contact and responding to peers with one-word answers."

Clue Cards:

  • Physical isolation at social times
  • Reduced peer interaction
  • Flat affect (no facial expression)







Station 2: Anxiety Around Personal Topics

Progress Note Snippet:
"In group session, Jordan changed the subject when the topic of family was raised; he tapped his foot rapidly and bit his lip."

Clue Cards:

  • Topic avoidance when discussing family
  • Nervous body language (foot tapping, lip biting)
  • Possible fear of disclosure







Station 3: Academic Engagement Decline

Progress Note Snippet:
"Jordan slept through parts of the math lesson, missed verbal instructions, and turned in only partial work."

Clue Cards:

  • Frequent in-class drowsiness
  • Incomplete assignments
  • Risk of falling behind academically







Station 4: Test-Related Stress

Progress Note Snippet:
"When asked about upcoming exams, Jordan stated, ‘I’ll fail anyway,’ and shook his head, expressing tearfulness."

Clue Cards:

  • Verbal self-defeat (‘I’ll fail’)
  • Emotional distress (tears, shaking)
  • Need for coping strategies around stress







Recording Findings:
Use the Clue-to-Note Mapping Worksheet at each station to document:

  1. Observation: What did you notice in the snippet?
  2. Inferred Need: What does this suggest Jordan needs?
  3. Related Note Component: (Observation, Intervention, Student Response, Plan)

Rotate stations until every group has completed all four analyses. Once done, reconvene for group sharing and mapping.
