Tier 2

Lesson Plan

Focus Check Blueprint

Students will learn to self-monitor their attention and behavior by pausing at key moments to record focus levels and reflect on strategies. By the end, they will independently use a simple checklist to track and improve engagement.

Self-monitoring builds self-awareness and teaches students to regulate their focus. Tier 2 checklists offer targeted support for those needing extra scaffolding, boosting participation and academic success.

Audience

7th Grade Students

Time

15 minutes

Approach

Model, practice, and reflect with a checklist.

Materials

  • Pause & Reflect Slides, - Attention Tracker Form, and - Self-Reflection Criteria

Prep

Teacher Preparation

5 minutes

  • Review the Pause & Reflect Slides
  • Print enough copies of the Attention Tracker Form for each student
  • Familiarize yourself with the Self-Reflection Criteria
  • Set up your projector or interactive board

Step 1

Introduction

3 minutes

  • Explain that today’s goal is to learn how to pause and check in on our focus during lessons
  • Display Slide 1 from the Pause & Reflect Slides
  • Ask students: “What kinds of things usually pull your attention away?”

Step 2

Modeling the Checklist

4 minutes

  • Show the Attention Tracker Form on screen
  • Walk through each column: timestamp, focus rating (1–5), notes on behaviors
  • Use the Self-Reflection Criteria to define on-task vs. off-task examples

Step 3

Guided Practice

5 minutes

  • Run through two sample content slides from the Pause & Reflect Slides
  • At each pause point, prompt students to mark their focus on their own tracker
  • Circulate and offer feedback, referencing the Self-Reflection Criteria

Step 4

Reflection & Wrap-Up

3 minutes

  • Invite a few volunteers to share what their tracker showed and one strategy they’ll try next time
  • Reinforce effective tactics (e.g., repositioning materials, brief mental reminders)
  • Collect trackers and remind students to use the form in upcoming lessons

Slide Deck

Ready to Track It?

Self-monitoring our attention and behavior during lessons helps us stay on track and take ownership of our learning.

Welcome students and introduce today’s goal: learning to pause and check in on our focus using a simple checklist. Display the session title and briefly explain why self-monitoring matters.

Why Does Our Attention Wander?

• What kinds of things usually pull your attention away?
• Turn to a partner and discuss one common distraction.

Ask students to think-pair-share about distractions. Prompt them to share examples of things that pull their attention away in class.

Introducing the Attention Tracker

Use the Attention Tracker Form:
• Timestamp: When you check in
• Rating (1–5): How focused you are
• Notes: What you notice about your behavior

Show the Attention Tracker Form on screen. Walk through each column: timestamp, focus rating (1–5), and notes. Reference the Self-Reflection Criteria to clarify on-task vs. off-task.

Practice: Pause & Reflect

  1. Watch the content slide.
  2. When you see the pause icon, record your focus rating and notes.
  3. Continue with the next slide and repeat.

Run through two sample lesson snippets. At each pause point, prompt students to mark their focus on their tracker. Circulate and offer feedback using the Self-Reflection Criteria.

Reflection & Next Steps

• Share one insight from your tracker
• Name one strategy you’ll try to stay on task

Remember to use this form in upcoming lessons!

Invite a few volunteers to share their focus ratings and one strategy they plan to try next time. Reinforce effective tactics and remind students to use the form in future lessons.

Worksheet

Attention Tracker Form

Instructions:
At each pause point during the lesson, record the time, give yourself a focus rating (1 = not focused at all, 5 = fully focused), and jot down brief notes about what you noticed in your behavior or environment.

Time (MM:SS)Focus Rating (1–5)Notes on Your Attention or Behavior

After the lesson, review your entries and reflect on patterns. Use the Self-Reflection Criteria to help you think about when you were on-task vs. off-task and identify strategies you’ll try next time.

Rubric

Self-Reflection Criteria

Use the table below to guide your review of the entries on your Attention Tracker Form. For each category, read the descriptors and select the level that best describes your reflection. Then write specific examples from your tracker and list one or two next steps.

Criteria4 – Exemplary3 – Proficient2 – Developing1 – Beginning
Identification of Focus PatternsClearly identifies multiple consistent patterns in focus ratings (e.g., times, topics, settings) with examples.Identifies at least one clear pattern in focus ratings with supporting detail.Attempts to find patterns but descriptions are vague or incomplete. No clear pattern identified in focus ratings.
Description of BehaviorsProvides detailed, precise descriptions of on-task and off-task behaviors, citing specific tracker notes.Describes on-task and off-task behaviors with adequate detail.Lists behaviors but lacks clarity or specificity about when/how they occurred.Unable to clearly describe on-task or off-task behaviors.
Connection to StrategiesThoughtfully connects observed patterns to effective focus strategies and explains why they help.Connects at least one pattern to a reasonable strategy with some explanation.Suggests strategies but connection to observed patterns is weak or unclear.Provides no or irrelevant strategies.
Action Plan QualityDevelops a detailed, realistic action plan with specific steps, timelines, and personal goals for improvement.Outlines a clear action plan with at least two specific steps to try next time.Proposes one or two general ideas for improvement without detail or structure. No concrete plan for future focus improvement.
