Pathways Intervention Plan

During daily 30-minute one-on-one sessions, the student will monitor positive behavior targets, record entries in the Daily Positives Journal, and analyze progress using the Goal Progress Chart to guide next steps.

Consistent progress monitoring and self-reflection help the student recognize achievements, build intrinsic motivation, and inform tailored supports, leading to sustainable positive behavior change.

Step 1

Greeting & Warm-Up

5 minutes

  • Welcome the student warmly and set a positive tone.
  • Ask: “Which positive behavior made you proud yesterday?”
  • Introduce today’s session goals and agenda.

Step 2

Review Behavior Goals & Data

10 minutes

Step 3

Reflection & Journal Entry

7 minutes

  • Prompt the student to verbalize what strategies worked and why.
  • Guide the student to record these reflections and successes in the Daily Positives Journal.
  • Encourage the use of descriptive sentences: “Today I did … because …”.

Step 4

Strategy Discussion & Goal Setting

5 minutes

  • Discuss “What can you try tomorrow to improve?”
  • Co-create a specific, actionable goal and record it in the Goal Progress Chart.
  • Note any changes to supports or strategies needed for next session.

Step 5

Closing & Positive Reinforcement

3 minutes

  • Highlight and praise one key win from today’s session.
  • Provide a chosen reinforcement (sticker, high-five, verbal praise).
  • Remind the student of the next session time and goal focus.

Display each goal, read it aloud, and point to the matching icon. Ask the student to recall examples of each behavior.

Review each symbol with the student and explain what it means. Encourage them to use these when marking future data.

Reflect & Journal

Ask the student to think about strategies that helped. Prompt with sentence starters.
