Matrix Columns & Definitions

  • Session Date: Date of the client session
  • Measure: Name of the outcome tool (e.g., PHQ-9, GAD-7)
  • Score: Raw or scaled result
  • Notes: Brief context or observations
  • Trend Alert: Flags significant change thresholds

Walk through each column one at a time. Clarify any trainee questions on definitions or where they might capture data in their own system.
