Mindful Transitions Plan

Students will learn to identify and manage emotions related to change through mindfulness practices, group discussions, and reflective writing, setting personal intentions for future transitions.

Helping students navigate emotional challenges during transitions promotes resilience, self-awareness, and a positive outlook, supporting mental well-being and successful adaptation to new phases.

Welcome and Warm-Up

5 minutes

  • Display slide 1 of Mindful Transitions Slide Deck.
  • Introduce the lesson objective: exploring emotions around change and setting intentions.
  • Invite students to share one recent transition they’ve experienced in one sentence.

Step 2

Guided Mindfulness Exercise

10 minutes

  • Advance to slides 2–4 in Mindful Transitions Slide Deck.
  • Lead students through a mindful breathing and body-scan practice.
  • Prompt students to notice sensations and emotions without judgment.

Step 3

Group Discussion on Transitions

15 minutes

  • Use prompts from Transition Discussion Guide.
  • Ask: “What feelings arise when you face change?” and “How have you coped?”
  • Encourage students to listen respectfully and support one another.

Step 4

Individual Reflection Activity

15 minutes

  • Distribute the Mindful Reflection Worksheet.
  • Students reflect on a personal transition: noting what they’re letting go of and hopes for what comes next.
  • Guide them to set 1–2 intentions for navigating future changes.

  • Invite volunteers to share one intention with the group.

Mindful Transitions: Letting Go and Looking Forward

Objective:
• Notice and manage emotions around change
• Practice simple mindfulness techniques
• Set personal intentions for future transitions

Welcome students. Introduce yourself and the lesson title. Explain: “Today we’ll explore the emotions that come with change, practice mindfulness together, and set positive intentions for transitions ahead.”

Understanding Transitions

What’s a transition?
• A shift from one phase or situation to another
• Can be expected (graduation) or unexpected (schedule changes)

Why it matters:
• Brings mixed emotions–excitement, anxiety, sadness

Briefly define transitions and invite 2–3 students to share one recent change they’ve experienced (e.g., moving to a new grade, a new school).

Guided Mindfulness: Breathing

  1. Sit comfortably with feet on the floor.
  2. Close your eyes or soften your gaze.
  3. Inhale slowly for a count of 4.
  4. Exhale fully for a count of 6.
  5. Repeat for 1–2 minutes, noticing each breath

Guide students through the breathing exercise slowly. Model each step and remind them to focus on the breath and let thoughts pass without judgment.

Guided Mindfulness: Body Scan

  1. Close your eyes and take a few breaths.
  2. Bring attention to your feet: notice pressure or warmth.
  3. Slowly shift focus to legs, hips, torso, arms, neck, head.
  4. Acknowledge any tension or comfort.
  5. If your mind wanders, gently return to the scan

Lead the body scan: start at the feet and move up. Pause at each area, naming sensations. Emphasize curiosity, not judgment.

Group Discussion

Discuss with a partner or small group:
• What feelings arise when you face change?
• How have you coped with a past transition?
• What supports helped you through it?

Divide the class into small groups or discuss as a circle. Encourage active listening and respect. Use additional prompts from the Transition Discussion Guide as needed.

Individual Reflection

On your worksheet:

  1. Describe a personal transition you’re experiencing.
  2. Note what you’re letting go of.
  3. Write hopes and possibilities for what comes next.

Distribute the Mindful Reflection Worksheet. Circulate to offer support or clarify prompts.

Setting Intentions

Choose 1–2 intentions to guide you through future changes:
• Be present with your emotions
• Practice daily breathing or body scans
• Seek support when needed

Encourage students to be specific and realistic. Offer examples: “I intend to practice 5-minute breathing each morning,” or “I will ask a friend how they’re doing when I feel anxious.”

Closure and Next Steps

• Share your intention with a classmate.
• Revisit your worksheet and intentions regularly.
• Use mindfulness practices beyond today’s lesson.

Invite 2–3 volunteers to share one intention. Remind students that mindfulness is a skill they can revisit anytime. Thank them for their openness and participation.

  • Listen Actively: Give each speaker your full attention.
  • Speak Respectfully: Use "I" statements and honor others’ experiences.

  • Be Present: Avoid side conversations or distractions.
