Lesson Plan
Consult Plan
Facilitate a focused 30-minute consult between teachers and the social worker to identify targeted accommodations and supports that boost engagement and prevent material destruction by a disengaged 6th-grade math student.
By collaboratively reviewing data and brainstorming evidence-based strategies, educators will leave with a clear, actionable plan to support the student’s academic participation and reduce disruptive behavior.
Audience
6th Grade Teachers and School Social Worker
Time
30 minutes
Approach
Data review + guided brainstorming to prioritize supports.
Materials
Prep
Prepare for Consult
10 minutes
- Review recent data on the student’s engagement and incidents using the Behavior Observation Form.
- Print 1 copy per participant of the Consult Agenda, Engagement Strategies Guide, and Implementation Action Plan.
- Familiarize yourself with the student’s math performance record and any existing IEP or 504 plan.
Step 1
Introduction & Context Setting
5 minutes
- Welcome participants and state consult goals: increase engagement, reduce material destruction.
- Review roles: teacher(s) share observations, social worker facilitates.
- Confirm agenda items via the Consult Agenda.
Step 2
Review Student Profile & Data
5 minutes
- Teacher presents academic performance, engagement patterns, and recent incidents documented on the Behavior Observation Form.
- Social worker highlights potential triggers and protective factors.
- Identify key moments when engagement drops or destructive behavior occurs.
Step 3
Brainstorm Accommodations & Supports
10 minutes
- Refer to the Engagement Strategies Guide for evidence-based ideas.
- Generate student-specific accommodations (e.g., tactile manipulatives, choice in problem types, scheduled breaks, check-in system).
- Discuss environmental modifications (seating plan, visual supports, peer buddy).
Step 4
Prioritize & Develop Action Plan
7 minutes
- Vote on top 3 strategies based on impact and feasibility.
- Use the Implementation Action Plan to assign responsibilities, set timelines, and define success metrics.
- Clarify which teacher or staff will implement each support and how progress will be monitored.
Step 5
Next Steps & Closing
3 minutes
- Schedule a follow-up meeting to review outcomes.
- Confirm communication channels for ongoing collaboration (email, shared document, weekly check-in).
- Distribute copies of all completed handouts and thank participants for their commitment.

Engagement Strategies Guide
This guide provides evidence-based strategies to boost engagement, reduce off-task behaviors, and prevent destruction of materials in a 6th-grade math classroom. Teachers and social workers can adapt these ideas to meet the unique needs of the student.
1. Instructional Differentiation
- Tiered Math Tasks
- Offer three levels of problems (basic, on-grade, challenge). Allow the student to select or rotate through levels.
- Tactile Manipulatives
- Use fraction tiles, base-ten blocks, or geometry shapes. Hands-on tools help maintain focus and reduce frustration.
- Visual Supports
- Provide step-by-step visual guides or anchor charts for multi-step problems.
- Highlight key vocabulary and symbols in color.
2. Environmental Supports
- Preferred Seating
- Position near the teacher or a supportive peer.
- Reduced Distractions
- Use seat dividers or noise-canceling headphones during independent work.
- Break Space
- Designate a calm-down corner with fidget items (stress ball, putty). Allow quick, scheduled breaks.
3. Motivational Strategies
- Choice & Voice
- Let the student choose the order of tasks or select from a menu of problem types.
- Goal Setting & Self-Monitoring
- Co-create daily or weekly goals (e.g., complete 5 problems with no material damage).
- Use a simple tracking sheet with smiley faces or checkmarks.
- Token Economy
- Award points or tokens for engagement, on-task behavior, and proper care of materials.
- Exchange tokens for small privileges (extra recess time, math game pass).
4. Behavior Supports
- Visual Behavior Cue
- Use a color-coded card system (green = good focus, yellow = caution, red = break time).
- Scheduled Check-Ins
- Brief (1–2 minute) check-ins at predetermined intervals.
- Ask, “How are you doing? Anything you need?”
- Repair Procedures
- Teach and practice steps for repairing or replacing damaged materials (e.g., ask for help, use tape, request new pages).
5. Social-Emotional Supports
- Relationship Building
- Start class with a quick one-on-one greeting or positive comment.
- Mindfulness & Calm Breathing
- Teach a simple breathing routine (e.g., 4-4-4 count) to use when feeling frustrated.
- Peer Buddy System
- Pair with a peer mentor who models appropriate engagement and care for materials.
Implementation & Next Steps
- Select 2–3 strategies from this guide to test in the next week.
- Record chosen supports in the Implementation Action Plan.
- Monitor progress through the Behavior Observation Form and adjust as needed.
Tip: Involve the student in choosing strategies to increase buy-in and ownership.
