Behavior Intervention Planner

Use this planner to guide your Tier 2 behavior intervention planning. Complete each section thoroughly to document data, goals, strategies, and progress measures.

Student Information

  1. Student Name:

  2. Grade:

  3. Date:

  4. Team Member(s):

1. Target Behavior Description

Describe the specific behavior you want to address. Include observable actions and context.

2. Baseline Data

What is the current frequency/intensity of the behavior? (e.g., number of occurrences per day/week)

3. Antecedents & Triggers

List known antecedents or situations that typically precede the behavior.

4. Consequences/Maintaining Factors

Describe what happens immediately after the behavior (e.g., attention, escape, sensory).

5. Measurable Goal

State a clear, measurable behavior goal (e.g., reduce occurrences by X% within Y weeks).

6. Intervention Strategies

Select and describe evidence-based Tier 2 strategies you will use to address the behavior.

7. Implementation Plan

Outline the steps for implementation, including timelines and setting.

8. Assigned Personnel

Who is responsible for each component of the intervention?

9. Progress Measurement

Describe how you will measure progress (e.g., data collection methods, frequency).

10. Monitoring Schedule

Specify when and how often the intervention and fidelity will be checked.

11. Notes & Adjustments

Document observations, challenges, and any adjustments made to the plan over time.

Rubric

Fidelity Check Rubric

Use this rubric to monitor and rate the fidelity of Tier 2 PBIS intervention implementation. For each criterion, circle the score that best describes the current level of implementation.

Criterion1 – Not Evident2 – Developing3 – Proficient4 – Exemplary
1. Implementation of Intervention StepsKey steps omitted or implemented irregularly. Some steps in the plan implemented; key elements missing. All planned steps implemented as designed.Steps implemented fully and refined for increased student engagement/effectiveness.
2. Data Collection AccuracyData are incomplete, inconsistent, or inaccurate.Data collection occurs but methods are inconsistent or contain errors.Data collected accurately and recorded per protocol.Data collected accurately, on time, and clearly documented with reflections.
3. Use of Evidence-Based StrategiesStrategies are not aligned with the documented plan.Strategies partially aligned; some evidence-based components missing.Strategies implemented as designed and match documented evidence.Strategies adapted to student needs while preserving core evidence-based features.
4. Consistency with Schedule & DosageMonitoring and intervention sessions rarely follow schedule.Sessions occur but irregularly; dosage may be insufficient.Sessions and monitoring occur per agreed schedule and dosage.Sessions exceed planned frequency with proactive fidelity checks.
5. Collaborative Follow-UpNo follow-up with team members or stakeholders.Occasional check-ins; limited communication of fidelity findings.Regular follow-up meetings to review data and adjust plan collaboratively.Proactive collaboration: fidelity findings drive timely adjustments and shared goals.

Scoring Guide:

  • Total possible score: 20 points
  • 16–20: Exemplary fidelity of implementation
  • 11–15: Proficient with minor areas for improvement
  • 6–10: Developing; needs targeted support
  • 1–5: Not evident; significant revision and coaching required
