Lesson Plan

One-on-One Coaching Plan

In this 30-minute one-on-one coaching session, the student will reflect on recent behavior, identify triggers and patterns, set a specific behavior goal using a goal ladder, and learn to monitor progress through a structured tracker.

This lesson empowers Tier 3 students to build self-awareness, understand behavior influences, and develop clear, measurable goals with ongoing progress monitoring, fostering positive behavior change and academic success.

Materials

  • Behavior Reflection Map, - Daily Success Journal, - Goal-Ladder Exercise, and - Progress Tracker

Prep

Review Fidelity Data & Materials

5 minutes

  • Review the student's Tier 3 PBIS fidelity data and any recent behavior incidents.
  • Familiarize yourself with the Behavior Reflection Map, Goal-Ladder Exercise, Daily Success Journal, and Progress Tracker.
  • Prepare discussion prompts based on identified behavior trends.

Step 1

Warm-Up & Agenda Setting

3 minutes

  • Greet the student and build rapport.
  • Briefly explain the session's purpose: reflect, set goals, and plan progress tracking.
  • Review session agenda with the student.

Step 2

Behavior Reflection

8 minutes

  • Provide the Behavior Reflection Map.
  • Ask the student to recall a recent challenging situation.
  • Guide them through mapping: situation, thoughts, feelings, behaviors, consequences.
  • Discuss insights and identify common triggers.

Step 3

Goal Setting with Goal Ladder

7 minutes

  • Introduce the Goal-Ladder Exercise.
  • Collaboratively define a specific, measurable behavior goal.
  • Break the goal into smaller steps on the ladder: immediate, short-term, long-term.
  • Confirm each step is realistic and time-bound.

Step 4

Journal Introduction

5 minutes

  • Present the Daily Success Journal.
  • Explain how to record daily reflections: triggers encountered, strategies used, successes, areas for improvement.
  • Encourage consistency and honesty in self-reporting.

Step 5

Progress Tracker & Wrap-Up

7 minutes

  • Share the Progress Tracker.
  • Define criteria for success and how often to review entries.
  • Set next check-in date and agree on support actions.
  • Summarize session highlights and express confidence in the student's plan.

Behavior Reflection Map

Use this map to think through a recent challenging situation and understand what led to your behavior. Fill in each section below.


1. Situation

Describe the specific situation or event that was challenging for you.






2. Thoughts

What thoughts were going through your mind at the time?






3. Feelings

How did you feel in that moment? (e.g., angry, anxious, frustrated, embarrassed)






4. Behaviors

What did you do or say as a result of those thoughts and feelings?






5. Consequences

What were the immediate outcomes or consequences of your behavior?






6. Triggers

Identify any triggers (people, places, times, events, or sensations) that may have contributed to your reaction.







Once complete, review your reflection with your coach to identify patterns and plan strategies for handling similar situations in the future.

Use this journal each day to track your behavior goals, reflect on strategies, celebrate wins, and plan for improvements.

Use this ladder to break down your specific behavior goal into smaller, achievable steps. Completing each rung will help you move steadily toward your long-term objective.


1. Define Your Behavior Goal

Top Rung: Long-Term Goal (by end of month)

3. Criteria for Success

Progress Tracker

Use this rubric to monitor and discuss progress toward your behavior goal. Review each criterion and identify which level best describes your current performance. Aim to move toward “Exceeding” over time.
