Wellness Survey Lesson Plan

Students will understand the purpose and confidentiality of a voluntary wellness survey and successfully complete it within 30 minutes.

This lesson is crucial for giving students a voice in mental health research, providing opportunities for school-based behavioral health support, and offering incentives for participation, contributing to a supportive school environment.

Materials

  • Survey Introduction Slide Deck, - Teacher Survey Script, - Internet-enabled devices (computers, tablets, or phones) for each student., - Projector/Smartboard for displaying the slide deck and survey links., and - Access to the internet for video and survey links.

Prep

Review Materials & Technology Check

15 minutes

  • Review the Survey Introduction Slide Deck and Teacher Survey Script to familiarize yourself with the content and flow.
  • Ensure all student devices are charged and connected to the internet.
  • Test the survey links (English Survey Link and Multi-Language Survey Link) and the video link on the classroom projector/smartboard to confirm they are working correctly.
  • Prepare to project the survey links clearly for students to access.

Step 1

Introduction and Overview

5 minutes

  • Begin by displaying the first slide of the Survey Introduction Slide Deck.
  • Use the introductory remarks from the Teacher Survey Script to explain the purpose of the survey, emphasizing its voluntary and confidential nature.
  • "Good morning/afternoon, everyone. Today, we will be completing a brief, voluntary, and confidential wellness survey for a research study conducted by Massachusetts General Hospital. Your participation is important because it helps us understand student well-being better and identify areas where we can offer more support."

Step 2

Video Presentation

5 minutes

  • Transition to the video slide in the Survey Introduction Slide Deck.
  • Say: "I am going to have you all watch the following brief video that will tell everyone more about the purpose, content, and importance of the survey. Please let me know if you have any questions."
  • Play the brief overview video. (Note: Since a specific video is not provided, the teacher should use a pre-selected video or describe its general content as "Brief Overview: Video").
  • After the video, ask if students have any initial questions.

Step 3

Understanding Post-Survey Options & Incentives

5 minutes

  • Display the slides detailing post-survey options and incentives in the Survey Introduction Slide Deck.
  • Using the Teacher Survey Script, explain the options for providing contact information for future research and connecting with school behavioral health support. Clearly state that confidentiality is maintained.
  • Explain the raffle for a $50 gift card and the requirement for LASID, reassuring students about data privacy even if they win.
  • "At the end of the survey, you will have the option to provide some contact information. This information is completely optional and will be used so that researchers at MGH can tell you about other clinical research projects for Newburyport High School students. You can also choose to be connected with further behavioral health support right here at our school. If you wish to be connected, only our school's designated point of contact will receive your student ID, and importantly, none of your survey responses will be shared."
  • "Additionally, students who complete valid responses will be entered into a raffle to win a $50 gift card. We will need your LASID (student ID) to enter you into the raffle, so we know who to give the gift cards to if you win. Even if you win, none of your survey responses will be shared with your school or parent/guardian."

Step 4

Accessing and Completing the Survey

10 minutes

  • Display the survey links slide from the Survey Introduction Slide Deck.
  • Guide students to access the survey on their devices using the provided links from the Teacher Survey Script.

  • Remind students: "If the survey page takes a few seconds to load, please be patient and wait, rather than refreshing the page. There's also a button in the top right corner of the survey for those who need to complete it in a language other than English."
  • Circulate around the room to offer support. "Feel free to ask me to define words or clarify questions as needed. However, remember that your responses must be your own to maintain confidentiality."

Wrap-Up

  • Allow students to finish the survey.
  • Collect any devices or paper if applicable (if they are using shared devices).
  • Thank students for their participation.
  • "Thank you everyone for participating in this important survey today. Your honesty and participation are greatly appreciated!"

A Brief, Voluntary, & Confidential Survey

Welcome students and introduce the topic. Emphasize the importance of their participation in a voluntary and confidential survey. You can use the first part of the Teacher Survey Script here.

What's This All About?

Explain that this survey is part of a real research study. Reiterate that it's voluntary (they can choose not to participate) and confidential (their individual answers won't be shared with the school or parents).

Direct students to watch the video. After the video, pause for any initial questions. Use the script: 'I am going to have you all watch the following brief video that will tell everyone more about the purpose, content, and importance of the survey. Please let me know if you have any questions.'

Your Options After the Survey

  • Optional Contact: Hear about other MGH research opportunities
  • Optional Support: Connect with school behavioral health resources
  • Your privacy is protected – responses are NOT shared.

Clearly explain the two optional choices students have at the end of the survey. Stress that choosing to provide contact information or seek support does not mean their survey answers will be revealed. Refer to the Teacher Survey Script for exact wording.

This survey is part of a really important research study conducted by Massachusetts General Hospital. Your participation is important because it helps us better understand student well-being and identify ways we can offer more support here at Newburyport High School. Remember, 'voluntary' means you can choose to participate or not, and 'confidential' means your individual answers will not be shared with your school or your parents/guardians."

"Before you begin the survey, I want to make sure you understand a couple of important options you'll see at the very end.

First, at the end of the survey, you'll have the opportunity to provide some contact information. This is completely optional. If you choose to provide it, this contact information will be used for two main purposes:

  1. Researchers at Massachusetts General Hospital can reach out to you about other clinical research projects that might be offered to Newburyport High School students.
  2. You can also choose to be connected with further behavioral health support right here at our school.

It's really important to know that if you wish to be connected with school support, only your student ID (LASID) will be given to our school’s designated point of contact – and absolutely none of your responses to these survey questions will be provided to them. Your answers remain confidential.

Second, for those who provide complete and valid responses to the survey, you will be entered into a raffle to win a $50 gift card! To enter the raffle, we will also need your LASID (that's your student ID) so we know who to give the gift cards to if you win. Again, I want to reassure you: even if you win the raffle, none of your individual survey responses will be shared with your school or your parent/guardian. Your privacy is a top priority."
