Wellness Checkup Guide

Students will assess their personal health habits across different wellness dimensions and set one actionable goal for improvement.

Understanding personal health habits empowers students to take ownership of their well-being and develop balanced lifestyles for long-term health.

Your Personal Health Dashboard (slide-deck), Habit Snapshot Survey (warm-up), Goal-Setting Stations (activity), and Reflection Radar (cool-down)

Prep

Preparation Steps

15 minutes

  • Review all generated materials: Wellness Checkup Guide, Your Personal Health Dashboard, Habit Snapshot Survey, Goal-Setting Stations, and Reflection Radar.
    - Print copies of the Habit Snapshot Survey and Reflection Radar for each student.
    - Prepare signs or labels for the 'Goal-Setting Stations' around the classroom (e.g., 'Nutrition Station', 'Movement Station', 'Sleep Station', 'Screen Time Station', 'Mindfulness Station').

Step 1

Warm-Up: Habit Snapshot Survey

5 minutes

Distribute the Habit Snapshot Survey and instruct students to quickly complete it, reflecting on their current habits. Emphasize that there are no right or wrong answers, and it's for personal reflection.

Step 2

Introduction: Your Personal Health Dashboard

5 minutes

Use the Your Personal Health Dashboard slide deck to introduce the concept of wellness dimensions (physical, mental, emotional, social) and how they contribute to overall well-being. Explain that the 'wellness score' is about balance, not perfection.

Step 3

Activity: Goal-Setting Stations

20 minutes

Guide students to circulate through the 'Goal-Setting Stations' activity. At each station, they will reflect on one area of wellness and brainstorm a small, actionable goal for improvement. Provide the Goal-Setting Stations Guide to help them structure their thinking.

Step 4

Share & Discuss Goals

10 minutes

Bring students back together. Invite volunteers to share one wellness area they focused on and a goal they set. Facilitate a brief discussion about the benefits of setting small, achievable goals for personal health.

Step 5

Cool-Down: Reflection Radar

5 minutes

Distribute the Reflection Radar for students to complete as an exit ticket. This will help them summarize their learning and reinforce their commitment to their chosen goal.

What's Your Wellness Score?

An interactive exploration of your personal health habits.

Ready to be your own wellness detective?

Greet students and introduce the overarching theme of wellness. Ask them to think about what 'health' means to them beyond just not being sick. Emphasize that this lesson is about personal understanding, not judgment.

Why Does Wellness Matter?

  • Feel your best: Have energy for school, sports, and friends.
  • Handle challenges: Cope with stress and tough situations.
  • Grow and learn: A healthy body and mind help you focus.
  • Live a full life: Enjoy everything you do!

Ask students to consider why paying attention to their wellness is important. Connect it to everyday life: how they feel at school, playing sports, or with friends. Highlight that being well helps them do more of what they love.

The Four Pillars of Your Wellness

Think of your wellness like a house built on strong pillars:

  • Physical Wellness: How you care for your body (food, sleep, exercise).
  • Mental Wellness: How you keep your mind active and learning.
  • Emotional Wellness: How you understand and manage your feelings.
  • Social Wellness: How you connect with others and build relationships.

Introduce the four main dimensions of wellness. For each, give a very brief, relatable example. For Physical: 'eating healthy, getting enough sleep, moving your body.' For Mental: 'learning new things, solving problems.' For Emotional: 'understanding your feelings, expressing them in healthy ways.' For Social: 'spending time with friends and family, helping others.'

Finding Your Balance

Your wellness score isn't a grade, it's a snapshot of how balanced your pillars are.

  • All parts of your wellness work together.
  • If one area is strong, it can help support others.
  • If one area needs more attention, it can affect the others.

Goal: Find what works for YOU to create a balanced, happy life!

Explain that true wellness isn't about being perfect in one area, but about finding a balance across all of them. Use a simple analogy like a bike with four wheels – if one is flat, the ride is bumpy. If all are good, the ride is smooth!

Goal-Setting Stations: My Wellness Boost

Instructions: You will rotate through different stations around the classroom. At each station, you will focus on one pillar of wellness. Read the prompt, reflect on your habits from the Habit Snapshot Survey, and brainstorm ideas for a small, achievable goal.

Choose ONE goal from any station that feels most important to you right now. Write it down clearly at the end of this guide. You will share this goal with the class.


Station 1: Physical Wellness

Reflect: Looking at your Habit Snapshot Survey, how are your habits related to sleep, eating, and moving your body? What's one small change you could make?

Brainstorm Goal Ideas:

  • Drink more water
  • Go to bed 15 minutes earlier
  • Choose a fruit for a snack
  • Take a 10-minute walk after school

Station 2: Mental Wellness

Reflect: How do you keep your mind active? How do you manage your screen time? What's one small way you could boost your mental energy or focus?

Station 3: Emotional Wellness

  • Practice deep breathing when I feel stressed

  • Spend time talking with a friend instead of just texting
