Goal Tracker Session

Guide the 8th grade student through a 15-minute one-on-one session to set a specific weekly goal, reflect on past progress, and plan aligned actions to foster responsible decision-making and accountability.

Regularly articulating goals and reflecting on outcomes builds self-awareness, accountability, and decision-making skills—key to social-emotional growth and academic success.

Step 1

Introduce Session

1 minute

  • Greet the student and explain the purpose: to set a weekly goal, reflect on last week, and plan actions
  • Remind them this is a confidential, supportive check-in

Step 2

Goal Setting

3 minutes

  • Ask the student to identify one specific, measurable goal for the week
  • Have them record it at the top of the Weekly Alignment Journal
  • Prompt for clarity: "What will success look like by Friday?"

Step 3

Reflect on Progress

4 minutes

  • Guide the student to review last week’s goal in the journal
  • Use the Self-Reflection Rubric to rate effort, challenges, and outcomes
  • Discuss briefly any barriers or supports they experienced

Step 4

Align Actions

4 minutes

  • Ask the student to brainstorm 2–3 concrete actions that will move them toward this week’s goal
  • Record each action in the journal alongside a target day/time
  • Confirm actions are realistic and specific

Step 5

Wrap-Up and Commitment

3 minutes

  • Have the student read their goal and actions aloud
  • Encourage them to commit to checking progress mid-week
  • Offer positive feedback and note any needed supports for next session
